The most common failure mode of incomplete [Role Transfer]. The operator informally delegates a domain, the hire makes a competent decision the operator wouldn’t have made, and the operator retrieves the domain — explicitly (overrides the call) or implicitly (starts doing the task again). Trigger is operator preference, not a standards failure. The hire registers the retrieval as the new boundary of their actual authority and stops taking initiative. Operator then complains the hire isn’t taking initiative — without seeing they trained the passivity in.
